Wisdom teeth removal

Wisdom teeth are the last teeth to come through, usually between the ages of 17 and 25. When there isn’t enough room, they can become impacted – stuck under the gum or growing at an angle – leading to pain, swelling and repeated infections. At Chalfont Dentist, Dr Naresh Hirani removes both routine and impacted wisdom teeth here at the practice, with the option of IV sedation if you’re nervous.

When do wisdom teeth need to come out?

Not every wisdom tooth needs removing. If yours have come through straight and are healthy, we’ll simply keep an eye on them. Removal is usually recommended when a wisdom tooth is causing problems, such as:

  • repeated pain, swelling or infection of the gum around the tooth (pericoronitis)
  • decay in the wisdom tooth, or in the tooth next to it, that can’t be repaired
  • food trapping and gum problems because the area is hard to keep clean
  • a cyst or other changes seen on an X-ray

We’ll examine you and take an X-ray. If the roots are close to the nerve in the lower jaw, we can take a 3D CBCT scan here at the practice. This shows exactly where the tooth sits, so we can plan the safest approach.

Wisdom teeth removal with Dr Naresh Hirani

Dr Naresh Hirani carries out the vast majority of our wisdom tooth removals himself, from straightforward extractions to impacted wisdom teeth that need a minor surgical procedure. He qualified from Guy’s, King’s and St Thomas’ in 2001, holds an MSc in Clinical Periodontology, and surgical procedures are a large part of his day-to-day work.

What to expect

  • Consultation – an examination, X-ray or 3D scan, and a clear explanation of your options, risks and costs before anything is booked.
  • On the day – treatment under local anaesthetic, so the area is completely numb. If you’re anxious, you can choose IV sedation to feel calm and relaxed.
  • Recovery – some swelling and stiffness for a few days is normal. You’ll go home with written aftercare instructions and a number to call if you have any concerns.

In the rare cases where treatment in hospital is more appropriate, we’ll explain why and arrange the referral for you. Patients can book directly, and dentists are welcome to refer patients to us.

No. If your wisdom teeth have come through properly and are healthy and easy to clean, they can usually stay. We only recommend removal when a tooth is causing problems, or is very likely to.

The area is completely numbed with local anaesthetic, so you’ll feel pressure but not pain. Afterwards, any discomfort is usually well controlled with over-the-counter painkillers for a few days.

Yes. With IV sedation you’ll feel deeply relaxed and remember very little of the procedure. You’ll need an adult to take you home and stay with you, and you mustn’t drive for the rest of the day.

Most people are back to their normal routine within a few days. Swelling usually peaks around the second or third day and then settles. Stick to soft foods, avoid smoking and strenuous exercise, and follow your aftercare instructions. After a surgical removal, it helps to take a day or two off if you can.

A dry socket happens when the blood clot in the socket is lost too early, causing increasing pain a few days after the extraction. It is more common in smokers. If it happens, call us – it is easily treated with a soothing dressing.

We’ll go through these with you at your consultation. They include swelling, bleeding, infection and dry socket. Because the roots of lower wisdom teeth can sit close to a nerve, there is also a small risk of temporary or, rarely, permanent numbness of the lip, chin or tongue. That’s why we use X-rays and, where needed, a 3D scan to plan your treatment carefully.
